Okay, let's break down one possible timeline of this relationship -
Beigh a shy, sensitive type and no danger to the fairer sex, Joshua's mother sets him up with a blind date for the prom with one of her friend's daughters (we'll call her Carla) who has her own set of peculiarities (I'm guessing overeating and an inferiority complex). They hit it off well, with Joshua being put at ease by Carla not being overly feminine and Carla is attracted to Joshua's sensitive nature ("when I talk to him, I feel like I'm talking to one of my girlfriends"). As can be expected, a sort of love develops and the expected marriage.
Fast forward a few years.
So far, the happy couple are childless. They explain the reason being that they don't wish to bring new children into the world when there are so many waiting to be adopted. Of course, they never adopt any children.

Joshua leaves Carla, with her consent and understanding, when he finally comes out. Of course no one is surprised. With her public fasade shattered, Carla drowns her dissapointment in food. Joshua and his new lover "Tad" buy a bar in Key West.

Carla has her stomach stapled, loses 300 pounds, gets a new beau, starts gaining the weight back, and loses her new beau to yet another man. Joshua has long split with "Tad" (the filthy slut) and out of his heartbreak found religion, joining the catholic church and training for entry as a priest.
That's just one possible scenario. But I'd lay good money that I'm at least 75% accurate.
